I realize it’s been a while since this thread was actively discussed, but I just found a problematic “echo” statement in your function “get_gravatar_url” — this breaks the image in some cases, and I think it causes your code to fail to display the image the first time it’s loaded. (The second and subsequent times it is loaded from the cache, so get_gravatar_url is never fired and the image is displayed just fine.)

Great stuff. I had to change a few things to get this working in the old PHP4 my hosting provider has. Also tinkered with styling a bit to make them look more like the original flares.
